<p></p><p style="text-align:center;line-height:27px"><br></p><p style="text-align:left;line-height:27px"><span style="font-family:宋体">各学院：</span></p><p style="text-align:left;line-height:27px"><span style="font-family:宋体"> </span><span style="font-family:宋体">根据学籍管理有关规定，“学生应按规定时间地点参加考核，因故无法参加者，经审批后可以缓考，缓考不及格者不再安排补考”。为使学生了解期末考试缓考申请相关事宜，通知如下:</span></p><p style="text-align:left;line-height:27px"><strong><span style="font-family:宋体"> </span></strong><strong><span style="font-family:宋体">（一）缓考申请条件</span></strong></p><p style="text-align:left;text-indent:28px;line-height:27px"><span style="font-family:宋体">符合下列情况之一的，学生可以申请缓考。</span></p><p style="text-indent:27px;line-height:27px"><span style="font-family:宋体">1.</span><span style="font-family:宋体">因事：学生因家中重大变故的，需要出具学院分管领导签字盖章的请假单。</span></p><p style="text-align:left;text-indent:28px;line-height:27px;text-autospace:ideograph-numeric"><span style="font-family:宋体">2.</span><span style="font-family:宋体">因病：</span><span style="font-family:宋体">学生因病确实无法参加考试的，需要出具二级甲等以上医院疾病证明，辅导员签字确认。</span></p><p style="text-align:left;text-indent:28px;line-height:27px;text-autospace:ideograph-numeric"><span style="font-family:宋体">3.</span><span style="font-family:宋体">因学校教育教学需要，如代表学校参加体育文艺竞赛、各种学科竞赛、外出短期学术交流等活动的，需要出具各类活动主办单位的通知及学院意见。整学期在外交流的学生不能申请课程缓考，需要返校申请补修，参加正常考试。 </span></p><p style="text-align:left;line-height:27px;text-autospace:ideograph-numeric"><span style="font-family:宋体">4.</span><span style="font-family:宋体">重补修考试安排冲突：申请时需写明两门冲突课程名称和考试时间。</span></p><p style="text-align:left;line-height:27px;text-autospace:ideograph-numeric"><strong><span style="font-family:宋体;color:red">5.</span></strong><strong><span style="font-family:宋体;color:red">特别强调：</span></strong><span style="font-family:宋体">未办理缓考手续，无故缺考者或考试违纪者<span style="text-decoration:underline;">取消补考资格</span>。 </span></p><p style="margin-left:0;text-align:left;text-indent:7px;line-height:27px"><strong><span style="font-family:宋体">（二）</span></strong><strong><span style="font-family:宋体">缓考申请时间及流程</span></strong></p><p style="text-align:left;line-height:27px"><span style="font-family:宋体">1.</span><span style="font-family:宋体">学生在考试过程中突发疾病，无法坚持完成考试者，可先行终止考试，并于本门课程考试结束后一日内凭疾病证明补办缓考手续。</span></p><p style="text-align:left;line-height:27px"><span style="font-family:宋体">2.</span><span style="font-family:宋体">因竞赛、交流需要，</span><span style="font-family: 宋体">重补修考试安排冲突的缓考申请缓考网上申请时间为：2023年4月23日12:00-2023年7月7日17：00，逾期申请不予受理。 </span></p><p style="text-align:left;line-height:27px"><span style="text-indent: 28px; font-family: 宋体;">3.</span><span style="text-indent: 28px; font-family: 宋体;">缓考申请需要登陆教务系统</span><a style="text-indent: 28px;" href="http://210.34.74.244/fjzyyjsxsd/">http://210.34.74.244/fjzyyjsxsd/</a><span style="text-indent: 28px; font-family: 宋体;">，按照附件《缓考申请教务系统操作说明》及以上时间要求进行缓考申请。</span></p><p style="text-align:left;line-height:27px"><span style="font-family: 宋体; text-indent: 28px;">4.</span><span style="font-family: 宋体; text-indent: 28px;">因事、因病、因竞赛交流需要申请缓考者网络申请送审后，</span><span style="font-family: 宋体; text-indent: 28px;">点击“打印”</span><span style="font-family: 宋体; text-indent: 28px;">，下载《福建中医药大学办理学习事宜申请表》，打印后附本通知要求的</span><span style="font-family: 宋体; text-indent: 28px;">相关证明材料</span><span style="font-family: 宋体; text-indent: 28px;">提交教务处明德楼</span><span style="font-family: 宋体; text-indent: 28px;">108</span><span style="font-family: 宋体; text-indent: 28px;">室后，等待审核。 </span></p><p style="text-align:left;line-height:27px"><span style="text-indent: 28px; font-family: 宋体;">5</span><span style="text-indent: 28px;">.</span><span style="text-indent: 28px; font-family: 宋体;">因考试安排冲突申请缓考者，申请上注明两门冲突课程名称和考试时间，交明德楼教务处</span><span style="text-indent: 28px;">108</span><span style="text-indent: 28px; font-family: 宋体;">后，等待审核。</span></p><p style="text-align:left;text-indent:35px;line-height:27px"><span style="font-family:宋体">附件:缓考申请教务系统操作说明</span></p><p style="text-align:left;line-height:27px"><span style="font-family:宋体"> </span><span style="font-family:宋体">福建中医药大学教务处</span></p><p style="text-align:left;text-indent:336px;line-height:27px"><span style="font-family:宋体"> 2023</span><span style="font-family:宋体">年4月23日</span></p><p><br></p><p></p>
